Specialist support coordinators help participants overcome barriers, manage crises, and develop strategies to achieve their goals. This service ensures participants are connected with the right supports and that their plans are implemented effectively, even in difficult situations.

Who Are We?

We are a Victorian-based, family-run team fluent in English, Greek, and Spanish, dedicated to providing personalized support coordination for participants.

Reach out via our enquiry form, phone, or email to express your interest in our Support Coordination service. Or, ask your LAC to send us a referral.

2. Initial Consultation

We call you (or respond to your email) to gather preliminary information about your needs and goals. This helps us understand how best to support you.

3. Documentation

If you would like more information or would like to proceed, we will send you the necessary documents, including consent forms, service agreements, and any other relevant paperwork.

4. Assessment

We'll conduct a thorough intake assessment to tailor our services to your unique situation.

5. Specialist Support Coordination Begins

Your Specialist Support Coordinator will be allocated and will reach out to schedule your first meeting.
